Does anyone know how to do a title page for a report?

I have to do a report for my medical terminology class and my healthcare law and ethics class. I have to do a title page, bibliography, and I think reference page. I haven't done these since high school which was six years ago so I don't remeber how to do them. I don't remember what to put on them, and spacing, etc. Could someone please tell me or give me a website with some examples.
