Does anyone have a form I can print?

Hi,

I am a student nurse and I have agreed to volunteer at a community health/wellness program this summer where I will be doing general health interviews/assessments and I was wondering if anyone has a general health history form that I can print out and use for the interviews? I think all the forms I have are more geared to a hospitalized patient and are way too specific. The people I'm working with will not be getting undressed or anything like that. I'm just going to quickly talk to them about thier general health and level of wellness, so something more simple would be better. I would greatly appreciate it!

Or you could use one of the free PDF-to-Word converters and save yourself some effort... or get hold of one of the free PDF editors and edit the document directly.
