In a word: Stupid.
Just like most people who drive sports cars they can't afford, this sounds like someone who is trying to make up for something that is woefully lacking in their lives such as average intellect, a normal sized ego, a caring maternal relationship, or adequately sized male anatomy.
If you are 18, and want to be called Doctor, then work hard, take the difficult classes in college, get terrific grades while working as a volunteer, knock the MCAT out of the park, go to medical school, do a residency, and become a real doctor. That way you can truly overcome whatever it is in your life that makes such a title so important to you (well....unless it is truly the latter reason stated above). Then, you can be a Doctor AND drive that little sports car without everyone laughing at you because you actually EARNED it!
