Doctor/Nurse Pairing vs Team Nursing

I have been in the same department for 16 years with a team nursing approach to our patient care.  I work in a inpatient/outpatient doctors office/clinic.  We have 6 different sites and the number of doctors/nurses varies at each site.  Some doctors rotate sites throughout the week others stay at one site all the time.  We have 10 doctors and 10 full time nurses plus one part time nurse and one per diem nurse.  At our largest site we provide patients with different procedures that requires a nurse at the bedside.  Not all doctors in the practice perform each of these procedures it varies.  The team approach has worked best keeping us all strong in all areas of the department.  Team nursing has been frowned upon by nursing here because it would change the care each one would deliver and how could one nurse cover for another when she hasn't done the procedure in months/years?  It is the doctors in the practice that bring it up every so often...or make comments about the lack of continuity of care.  Looking for feedback in other clinic/MD office settings if you do pair/team nursing in a practice that functions more like an acute care area and how it goes for you.  Did you ever trial a different option in your practice.
