I got into nursing school and I need to hand in health forms. It requires me to get vaccinations (new) and write down dates for the old vaccines I got. I called my old pediatrician for my files so that I can transfer them over and they said they are looking. are these files necessary?

The files are only necessary for your own information. The information you place on the form or that is placed there at the doctor's office at the time of the school physical is sufficient. I showed my shot records each semester when we had to show our stuff again before going to clinicals. The physicals, both for school and employment, I have always got at urgent care clinics as a matter of convenience.

You need to have new physical regardless - however, there are a couple of things to note; If you don't have a record of your immunizations, you'll have to have a titers test done as well, which measures your immunity levels for all the diseases you'd normally have immunization shots for (basically verifying that you've most likely had these immunizations). That can be done during your appointment for your physical. But it takes a few days to get that back...so make sure you don't wait until the last minute.
Get your shot information put into one of the yellow shot record forms. Those are easier to carry around and can be used for their original purpose, travel. If your doctor's office doesn't have the blank forms (most doctors have these forms), you can get a blank one at the county health department.
