Disturbed Sensory Perception: Visual Vision Loss

I had a pt with end stage MS and I have to make a full scale care plan which includes a nursing diagnosis relating to each functional health patter. I'm currently doing the cognitive/perception FHP and my client is A/O x 3, has no vision or hearing problems, but is confined to a wheelchair (this last point probably doesn't apply to what I'm asking)...In reading about MS patients, I found that they are at risk for developing impaired vision. My nursing nursing diagnoses book/'bible' lists this:

However, it does not indicate that it can be used as a 'risk for' dx. Can I convert that to a 'risk for' dx with something like this: Risk for disturbed Sensory Perception relating to impaired vision secondary to MS?

I know this doesn't really help, but I've found that some instructors let you create "Risk for..." from any NANDA. Others on the other hand, only let you use actual Risk for NANDA's that are already created and listed. I always just ask my instructor at the time to see what they prefer.
