Disclosure

So I know that you are supposed to disclose any criminal record, but what if you don't remember your exact charges? There were a few times that I was in a holding cell, released, paid a fine, and then never received documentation or a court date. This happened 6 and 10 years ago. Should I contact those counties and request a background check? Or just disclose that there is one and leave it at that? Should is disclose charges that have been dismissed?
