Diploma Verification Form

I am trying to apply to take my NCLEX in California. I have all my documents except for the Diploma Verification Form. 

But on the list of deficiencies it says I DO NOT need to submit this form if I have my Philippine nursing license, if I am understanding correctly.

Has anyone been able to take the NCLEX by just submitting their license?
