My patient has an order for Ddavp if urine output is more than 100ml/hour. So when she voids I would be averaging out the volume over the length of time since previous void correct? This is what I believed but another nurse felt it was only during the specified hour of the void. So if she voids 250ml then we give Ddavp regardless of how long since previous void. The order also specifies there must be one void in between doses and that the specific gravity has to be less than a specific amount.

Definitely the way you described. Depending on this patient's age and weight, voiding over 100mL in a single occurrence may be perfectly normal. (Remember that normal urine output is 1-2 mL/kg/hr.)

Your way is the only way that makes sense. If the MD wanted it to be based on amount of urine per void the order would say "if voids over xx amount" or "over xx amount per void"...unless this nurse thinks that this pt actually takes an hour for a single void hence the "per hour?"
You should clarify since people are interpreting and medicating differently but the way it is written your way is the only way that makes sense.
