I created it when I was working on something to help allnurses students with assessing and learning about medical diseases and how that information fits in with nursing care. It's just something out of my mind that I came up with after years of practice and finally realizing how these pieces fit together. I've been trying to figure out how to merge it with a worksheet on how to write a care plan--here in the U.S. the nursing schools want the students to follow the nursing process when writing care plans and many students have difficulty with that. Tell your instructor and any other students in your school that they are more than welcome to copy and use it. It has currently been downloaded more than 7,000 times according to my attachments page. I posted it on allnurses in order to share it with everyone. You can also download and copy it from here: . If it helps make information easier to process, then it will have accomplished its purpose.
