what date do I put for an assessment on a discharge/transfer??

I have been putting the date on the transfer assessment as the date that I fill out the form but keep hearing from the office that this is wrong.. They always call in the middle of my already busy day to ask about the dates.. And, the problem is, I never know right then cuz I am driving all day what the date was... Then, they are all rude about it... But, what date is it? Is it the date I did the paperwork??? The date I last did an assessment??? M0090 is the one I am talking about..

According to OASIS Central (emphasis is mine):

M0090 is the last date when the final assessment data are collected. For the SOC assessment, this must be completed within five days of the SOC date. In most situations, assessment data is completed in association with a patient visit and M0090 would be the last visit date in which assessment data was collected.

The date of the transfer is the date that the agency learned of the transfer. If we're notified of hospital admission two days after the fact, then that date is what is put for the transfer date. The date of the assessment is the last date that the patient was seen by someone from the agency.

M0906 - The transfer date is the date that the patient is admitted to an inpatient bed - not ER, or for diagnostic purposes. Not the day you are doing the OASIS.

For discharge - M0906 is the day of discharge.

M0090 - is the date that the clincian performs/completes the OASIS assessment - may or may not coincide with a visit. For a transfer, needs to be within 2 calendar days of knowledge of transfer. Is different from the 5 days for recert and SOC.
