DAR charting homework

I have to write a short Data Action Response (DAR) on a patient for class and I am not sure how it should read...Can anyone give me an example? :o) Thanks!

[color=#483d8b]we have computer charting, and while we don't use "dar," we use other words that stand for the exact same things. when we open up a "nursing note," we get the d,a,r along the side and are expected to type in the appropriate information. remember, in nursing documentation, complete sentences/paragraphs are not necessary.

i do not have a sample to show you. samples are hard to come by so i'll have to make one up. however, dar charting is one part of something called focus charting that hones in on a patient's needs. recall that maslow organized needs into a hierarchy. gordon sorted them into 11 classifications. or, the need could relate back to a goal or outcome from the written care plan. what did your nursing program tell you to do and how to classify or organize patient needs? i ask because your charting will be based on this concept.

dar is an anachronism that stands for

(d)ata--(a)ction--®esponse

another componenct of focus charting is dae which is

(d)ata--(a)ction--(e)valuation

when charting with this format or in this way you follow the sequence of the letters in the word "dar". your data is the subjective and objective information you gathered or observed about the patient in regard to a particular need that support the proof that the need exists. your action is any nursing intervention(s) you performed in relation to treating and/or addressing the need. the evaluation is your analysis, try to keep it as factual as possible, of how your patient responded to your actions.

here is an idea of how this kind of charting note might be put together:

need: comfort (or, relief of pain)

d - complaining of continuous, sharp pain in mid-abdominal incisional area. crying. "i need something for pain now!" states pain is 9 on a scale of 10.

a - medicated with demerol 75mg im in luoq of left buttock. repositioned on right side with pillow to abdomen to help splint wound.

r - patient stated pain was "much better" 30 minutes later and rated it 3 on a scale of 10.---n. nurse

another way to write this is is to just put all the sentences together:

comfort (or, relief of pain) - complaining of continuous, sharp pain in mid-abdominal incisional area. crying.
