Daily newborn assessment form

I am an OB nurse in a small rural community hospital looking to update our daily newborn assessment forms. Wondering if anyone has any samples of simple newborns assessment forms. I know a lot of hospitals are going to computerized charting now, but our hospital is years away from making that advancement and we are doing a lot of double and triple charting on our newborns as well as post-partum mothers. So any ideas would be helpful!:nurse:

Ours is a flow chart as well as has area for narrative. I can try to scan it and send it to you, if you want. I don't work til Friday however. Good luck in finding what you need.

I would greatly appreciate a copy of that Newborn assessment form. I am looking for a chechlist type form to move easily from one assessment to the next. Anything to build from would be great.
