Okay, I need your help guys. I filed my CVS last August, got a notice of complete application in September so I thought everything is okay now. To my surprise, I got a notice of incomplete application thru email from NYSED just now.
They said I lack the ff:
1. Official transcript sent directly from your school to this office.
2. Photocopy of your diploma.
I am devastated. After 10 months of waiting, I will get this news? Now I need to go back to CGFNS and send some forms because apparently, they didn't receive this form from me.
TRANSCRIPT VALIDATION FORM FROM MANILA DOCTOR'S COLLEGE 06/2006 - 03/2010WAITING TO BE RECEIVED [?]
TRANSCRIPT AUTHORIZATION FORM FROM MANILA DOCTOR'S COLLEGE 06/2006 - 03/2010RECEIVED [?]
Now my my question, is the transcript validation form same as the authorization for verification of academic records/transcripts?
Also, I wonder how are they gonna send the photocopy of my diploma since there's no such form to accomplish for that? Is it okay to attach the letter from NYSED along with the other form?
I don't know where to start. Now I have to pay another 175USD because my application will expire next month and I doubt that everything will be done in a month.
Any form of help will be appreciated. Thanks guys!

Both. The two forms are all included in the CVS handbook. You should send them already.. the wait is too long.
