CVS Application for New York - Question

We came across a small problem with the CGFNS online application. After we fill all the details and reach the step where we print the forms in the automatically generated pdf files

1. Validation of license/registration form has no license issuing authority/address.

2. In the application form, the license information again comes up blank.

I cannot even seem to find the page where we are supposed to enter the licensing issuing authority name and address. If we don't fill that up, it comes up blank, so I want to fill it up but I just don't know where. None of the steps had a page where you fill in license issuing authority information, there is a page where we fill in our own license information but that page doesn't ask for the issuing authority information

Did you face this same situation when you filled yours? Do any one who did an online application remember if you had to enter this information manually?
