Criminal Records in NYS/CT/NJ What do they ask for?

For those of you successfully licensed despite having criminal records, what paperwork did you provide to the BON of your state? The Deposition of Indictment? Letters of recommendation? Any other specific court documents? 
I am planning ahead. Thanks in advance.
