Concept Mapping

This semester in clinicals we are using concept mapping and I don't have a clue on how to do one. My pt diagnosis is Atrial Flutter, Atrial Fib, Dysrhythmias, COPD. Any suggestions? I wish the instructor would let us see a sample of one of them.
