competency objectives

hi all

i am a third year student undertaking a nursing home placement. the problem i am having is my log book. my facilitator is telling me i waffle too much and need to simplify my objectives, strategies and evidence. can anyone give me a guideline :banghead:

I guess I'm confused on where your focus is SUPPOSED to be...are we talking careplanning or general goals for each clinical day?

hi.

I have a log book that contains the 10 competencies. in this we have to have objectives we want to achieve, strategies on how to achieve them and evidence (Signed by RN on duty that we have achieved it) my facilitator in the first week signed them off and said they were great now he says i have put too much information in them. so for example Competency 10 - 'Collaborates with the interdisciplinary health care team o provide comprehensive nursing care'

objective isto work effectively with residents, allied health team to provide nursing care. my strategis are for eg. documenting relevant information as soon as possible regarding care given, ask other staff for asistance for residents needs,liaising with doctors other staff and resient/family for good outcomes. evidence would be assisting and follwing RN direction, recording actions taken inprogress notes and working and communicating with all allied health professionals. hope this clarifies it more. my facilitator says this is too much info hope you can help
