Compency evaluation tools

I am trying to develop a competency evaluation tool to use for our new nurses prior to starting their oriention. Something that can help determine what they will need in orientation and what areas need targeted. If you have any information please let me know.

We have an approx 4 page checklist that they need to go through before they complete their orientation. Is that what you are looking for?

I would like to see the checkoff list that you have if I could. I still want some sort of tool to see what level the nurse is at and what topics do we need to really cover in orientation (besides the basics). I am the new Med Surg Director of a 35 bed hospital and I am developing new checkoffs, evaluation tools, and competency check. I welcome any information on these topics for a hospital setting.

My Ed Dir is out on vacation this week, and I can only get paper forms at this time as the computer type is on her computer. I am at a LTC facility though.
