having to come in to sign forms

Hey guys,

Nursing is my second career. I have been lurking on here. I have been a rehab nurse for a few months. We have to do FIMS and other forms such as adult ongoing assessment. I wanted to ask you this important question. Do you have to come in on your off day or immediately (stat) to finish a form that you forgot to sign off? This has happen 2 times to me already. The supervisor emailed me and told me that I forgot to sign a form and that I have to come in stat in order to do it. Today I couldn't because my husband forgot to leave the car keys at home. (We only have one set of key, lost the spare set).

I was really mad. I told her that I would come in to sign in when I have a chance which is the next day that I work. The first time I told her I can come sign it when I'm off. She insisted that I come in right away to sign it as the patient are discharge and they might lose some reimbursement? Other nurses told me they just sign it whenever they go back to work and not on their off days. I don't know if I'm a new nurse here that is why she is doing that. I'm really ticked off. I had to drive in at 9:30 pm to sign some of the stuff. The first tiem I had to wake up 8am in the morning on my off day to come in and sign them. The first one was just a breathing treatment that I forgot to sign off.

FIM scores are how your rehab unit gets reimbursed, so its pretty important for all documentation to be finished/signed/etc, so it can be submitted in a timely manner. If your unit ever has RAC audits, volunteer to participate so you can see just how important it is for things to be signed and for dates and times to match. Medicare doesn't mess around when it comes to seemingly little things like dates, times, and signatures.

Due to your use of the 'adult ongoing assessment' ACEIT terminology, I conjecture you work at a HealthSouth Rehab Hospital. If my guess is correct, you can access and complete your charting remotely from home using the remote HealthSouth website. It will keep you from having to make trips to work on your time off.
