Colorado criminal background documents

Had a question for anyone who applied for Colorado license with criminal history. 

The application asks for supporting court documents. Can anyone explain more specially from their experience? Like...just the court findings or also the police report from the arresting officer?

I have a DUI from 2017...and I wasn't the nicest person during the arrest (I'm mortified just thinking back about it) and I'm terrified of ANYONE reading just how rude I was.

So I had a dui with an accident 6 years ago. I submit the police report, the court papers showing what my punishment was (license suspension and I got PTI for the accident which was a felony but dismissed when I completed the PTI), I wrote an explanation letter explaining what happened, how I regret it and how I’ve made changes in my life. I also had a couple of my instructors that I made aware of my background write me reference letters. Also, if you completed any type of rehab or IDRC I would send that also. I got mandated by the court to complete a 2 day IDRC class so I just asked them for a letter that I was compliant and confirming the date I was there and I sent that. I’m in NJ but I’m guessing the process is similar for all states. Took about a month, but I was issued an unrestricted license. Trust me, I know it’s embarrassing but I’m sure the board has seen many things. I was super embarrassed and ashamed of what I did, but nurses like everyone else are human and make mistakes too at the end of the day! I hope this helps, prayers for you!

I’m in CO. I had a dui in 2019, while I was in my second semester of nursing school. I submitted a copy of the ROA, final sentencing, DUI class and therapy completion, MADD VIP certificate, Community service completion form, and masterdrive class completion. 
 

My police report was also horrible and I did not want to submit, so I didn’t. I was planning to if they requested it, but the board made me do a MSE interview with the Peer Assistance program, and no one ever asked for it. 
 

I had to wait about 2 months after taking the NCLEX, but I ended up with an unrestricted license.
