collaborative problem

Each part of a collaborative problem focuses on different aspects and concerns.

Say, Ms. Smith was admitted to the hospital for lung CA.

The doctor would be concerned with the aspect of diagnosing her with lung CA and prescribing treatment such as chemo, radiation, etc.

A nurse would be more concerned about her response to the disease and would use a nursing diagnosis such as ineffective breathing pattern or chronic pain.

Both health care providers are working to help Ms. Smith achieve her optimal state of health, but they go about it in different ways.

okay i understand that, but i am still somewhat confused. can you or anyone tell me whether or not thses statements are a collaborative problem or nursing diagnosis. constipation related to irregular defecation habits. is it a cp or nd and explain? risk for cardiac dysrhythmias related to potassium deficency? hypoxia related to chronic obstructive pulmonary disease?

Collobarative problems are headed with PC for potential complication. An example would be PC: Sepsis or PC: Acidosis (metabolic). With a PC dx there will be or should be nursing dx that would fit with it also. A nursing dx should not have a medical term in the r/t field...unless possibly when in use with a secondary. For example Risk for injury r/t altered mobility secondary to arthritis. Your nursing careplan book and/or nursing fundlementals book should be able to guide you.

thank you also for breaking thing down for me. I am glad to be on this website i can across it by mistake thank my lucky stars. i'm almost 50yr and just starting the transition class march the 25th to be exact. I am scared, but i can taste my RN license.
