cna replacement license (ga)

got my cna license in high school in 2004. And never received it how can I get repalacement copy. Teacher said that they mailed it twice, but I have not received it. Can some one please help me.

do you need a new replacement for a lost card?do you need reciprocity from another state or do you just need you very first card? I had written down a website to go to for my reciprocity but......I cant find it at the moment(what help would that give ya?)we are in the process of moving into a house here in Macon,Georgia and things are all boxed up......no tellin' where its at!!!!!!!
