CNA license for Nursing Students

Published

Hello, I've finished fundamental nursing and interested in applying for CNA certification in Northern California. I went through the CPDH website and Credentia.com but haven't figured out the correct process. Here is what I am assuming: Send official transcripts, the form 283E, and Live Scan to CPDH then get the form CPDH 932 in order to apply for the testing at Credentia. I'm not very sure how it work. Thank you!

The information is on this website:

Fill out this form: https://www.cdph.ca.gov/CDPH Document Library/ControlledForms/cdph283e.pdf

Complete this form and take to a live scan center near you:https://oag.ca.gov/sites/all/files/agweb/pdfs/fingerprints/forms/BCIA_8016.pdf

Here is a sample Live scan form:

If you're currently in nursing school, get your transcripts. According to the CDPH, "Proof of work is not required for current nursing students or if the college degree was obtained within the last two (2) years."

Submit to the mailing address on the CDPH 283 E form. That's the first form.
