I am currently doing an assignment that requires me to interpret physical assessment findings to estabish a foundation for nursing care for a patient with food poisoning. I need to identify, and apply Gordon's Health Patterns; Identify 10 nursing interventions-apply rationals for the top 3 and include relevant discharge planning information. I have the interventions covered along with the rationals and discharge planning. What I'm having difficulty with is documenting how Gordon's apply to the case study. If anyone could guide me in the right direction I would be truly grateful.

here are three websites where you can get very good information on how to assess a patient by gordon's 11 function health patterns.

what you want to do is answer the questions these websites provide under each of the 11 areas. from them you specifically want to highlight the answers that are abnormal responses. this means that you must know what the normal answer should be.

you are saying that you already have your interventions and rationales done. don't know how you accomplished that if you haven't determined what your patients problems are yet. your patient's problems are determined by the abnormal assessment data you get when you do an assessment by gordon's 11 function patterns. this is telling me that you did an assessment of this patient by some other way than gordon's functional patterns. that is not what you were supposed to do.

to correct this you have to pull up the information on the three websites i posted for you and answer the assessment questions under each of the 11 categories for your patient. then, and this is the important part of the assignment, pull out the abnormal responses, because these are your patient's problems. do they match with the ones you already determined? did any new ones turn up?

now, each of these abnormal responses that you found becomes a nursing problem that you can develop nursing interventions for along with the rationales. you incorporate the pathophysiology of food poisoning along with the rationales for the nursing interventions to tie everything up into a nice logical presentation. the "other" rationale to this assignment that your instructor(s) are looking to see that you are getting (and you totally missed the mark on) is that you found your patient's nursing problems through your assessment and data collection activity. your instructors are specifically wanting you to assess and collect data on your patients using gordon's functional patterns, not by whatever way you did do it.
