Clarification

Hi everyone I am trying to construct some care plans for a patient with diverticulitis and I came up with these:

PC:hemorrhage secondary to diverticulum herniation and perforation

ND:Impaired tissue intergrity related to gastrointestinal perforation and peritonitis

My question is are both of them talking about the same stuff because during my research they both came up with the same interventions,could I be doing the wrong care plan? please help me out
