I'm writing a careplan for a 23 month old child with MRSA. My diagnosis goes as follows; Impaired skin integrity r/t damaged skin layers secondary to presence of pathogens (MRSA) am/b lesions on right labia, upper right posterior thigh, lower abdomen, with purulent exudate, and positive MRSA culture.

I have to write short term and long term goals. I normally would do education for my short term goal, but she's too young to understand that, so I'm having trouble with that. Can I educate the parent for the short term goal? I'm lost as to how to set my goals for a child this young. Please help! Thank you!

That's a pretty long diagnoses. Are your instructors requiring the 3 part statement with the "As evidenced by" or you are a thorough person. Either way, if I were in your shoes, you could consult your nursing diagnoses book for the goals if you're having trouble. The NOC outcome in mine says," skin mucous membrane will be intact AEB by following indicators...." To make it patient specific in your circumstance, you could say, "Patient's skin and mucous membranes will be intact within 7 days." Or, something like that. Make sure to have a measurable time line. I'm not sure what you mean by education. As in, "patient will report S/S of skin infection by discharge?" Probably something a long those lines. Sure, teach the family otherwise. "Patient's mother will report and understand the S/S of a MRSA skin infection upon discharge." I don't know. Just giving ideas. Good luck.
