I am having a complete brain fart about how to chart via teach-back method. I haven't charted in over 6 months so I feel rusty.
I taught a patient how to use his home B/P machine today and he demonstrated it back to me twice.
Is this this correct: Educated pt on how to use home B/P machine. Pt demonstrated using home B/P machine twice by placing cuff on correctly, turning machine on, and verbalizing systolic and diastolic numbers.
sounds fine. I would just state patient able to demonstrate use of BP machine and verbalized understanding of results. You will probably formulate a sentence that applies to everything eventually. Good going!
