Certificate of Employment of PH nurse for Canada PR application

Hi. I am in the midst of completing documents in preparation for PR application in Canada and I would greatly appreciate if you can send me a sample of the COE you obtained based on the requirement of CIC. It's the same as what regulating bodies require.

The COE should indicate specific employment period with the company, position held during the employment and time spent in each position (mine is 40hours per week, 5day work per week), main responsibilities and duties in each position and total annual salary plus benefits, the number of hours worked per week (which i have mentioned above).
