Large balloon on the foley is applied with traction (usually adhered to the upper thigh in some way) so that the foley remains in the prostatic fossa (the area that had prostate and is now empty) and helps aid in hemostasis. They usually dont recommend traction after 24 hours to avoid necrosis since the balloon is compressing vessels underneath.
