Caseloads question

Case managers, how many days do you work and what is your current caseload? How often do you see each patient on average?

Hi Butterfly. We carry anywhere from 11-24 patients depending on census per RN case mgr. We have a corporate policy to make at least 20, preferably 25 visits per week. Corporate has earmarked certain diagnoses as high risk for revocation. These are the visits we have to make at least 2x week if not 3.

That's interesting about both policies. How long is a typical visit ?

A minimum of 15 min. Usually they are 30-45 minutes.

We typically carry a caseload of 12-18 patients and each nurse is required to make at least 16 visits per week. Another RN and myself share an LPN who assists in making the required visits. I usually see each patient once per week for a routine visit and then also for any pain crises, falls with injury, emergent situations, deaths or admissions. A few years ago, I worked for a company that required 20+ visits per week and it was pure hell. Ended up with 55-75 hr work weeks!

That's interesting about required visits. Our company does not require nurses to make a certain number of visits per week, we like to visit each patient once per week and as needed. Case loads are about 20-25 people. I have 15 but i only work 3 sometimes 4 days a week, and am actually a PRN that's been covering patients till we get a case manager trained. Visits including documentation are about 1.5-2 hours and travel time is 30-60 minutes between houses (we live in a very large rural area).
