For those doing insurance case management in the home, for example in a transitions program, what does the company look for in a care plan? Trying to come up with one for an interview.

I came across your question while searching for interview questions for Insurance Case Management. Can you give me an idea of what sort of specific questions you were asked?

I do not have a specific care plan software or program from Hosp, SNF, rehab to home. I am not a TCM (applying now) but I have worked in rehabilitation hosp, HHC mostly as a lic. PT Assistant, but also RN a few years ago.

I think they are looking at knowledge of specific dz process(es), dz or d/o mgmt ie., HTN, diet, medication compliance, DME needs, community resources, safety barriers to prevent injury (dementia, abnormality of gait/generalized weakness--ding ding PT or OT if ADL assist), if Medicare they need a face to face with PCP within 30 or 60 days.

Maybe this: Infrequently happened but more than you think-- pts that go home with DC MAR/med list with meds at home per PCP--can lead to confusion, lack of compliance until sees PCP, or overdose. It is not always covered on initial HHC RN assessment.
