Care Planning

Do any of you have, or know where I can find, nursing care plans specific for Hospice care?

Hospice plans of care are individualized and I am not aware of a resource for them other than general nursing resources. They typically revolve around symptom management (ie: N/V), alterations in comfort or skin or nutrition or well being, knowledge deficits, etc. Some hospice software have basic templates in place...so you can choose, say alteration in skin integrity, customize it to say due to immobility, set short and long term goals with anticipated end dates (if you want) and then either select typical interventions from a list or create your own unique interventions.

My POCs are electronic now, and I have drop down boxes for the standard verbage. I like this as it is generally faster and adequately flexible to make the POC reflective of each individual patient. When I was on paper I kept my own little pocket book of nursing diagnosis to help with my language and had notes for my pet problems etc.

If you are on a laptop but your digital record doesn't have pre loaded nursing diagnosis, you can try downloading sample plans at RNcentral.com and saving them to a special file where you can cut/paste them into your document.
