Care plan/ work load question

What does a care plan entail in your school?

Here's what I have to hand in tomorrow (and am only 10% done with :( ).

1) Face sheet (biographical data etc)

2) Hx of present illness w/ pathophysiology explained and medications.

3) Hx of past illness w/ ditto

4) Subjective and objective data for all of the 11 functional health problems

5) 3 nursing diagnoses, w/ expected outcomes, interventions and rationales

6) 1 collaborative problem

7) Problem list w/ relevant subjective and objective assessment data (including meds and labs) and all the dx

8) Medication list w/ adverse effects & drug interactions, nursing assessments, client teaching and evaluation of effectiveness in this patient

9) Cultural assessment

10) Teaching-Learning Plan addressing the 3 domains of learning

11) Discharge plan

12) Evaluation of an educational resource

13) 2 articles from peer-reviewed nursing journals

14) APA format reference page for these 2 articles

Is this more than usual for a care plan? Or is it just me not coping?

nope pretty much a normal care plan. I am finishing one up tonight myself. sucks to be a nursing student!
