I'm doing a care plan on a 27day neonate who has herpes simplex 2. I'm stock at the level of nursing intervention and Evaluation. Can someone help

first look at your assessment and see what is going on with the patient. look in your textbook about effects of hsv on newborn and match it against your assessment of the neonate.
is the pt. exhibiting signs of pain? is there impaired mucous membranes? impaired tissue integrity r/t lesions? is there temp instability? poor suck/swallow? s/s of infection or encephalitis? risk for injury...pain....impaired nutrition...ineffective thermoregulation are just a few possibilities. it all has to start at what is going on with the patient at assessment.

Thanks. The child had low temperature, poor feeding and was diagnose HSV 2 menigoencephalitis.He been treated with prevacid and acyclovir

unfortunately, i can't help you. you aren't understanding how the nursing process and care planning works. we have to know what this patient's nursing problems and the signs and symptoms of each problem are that go with them in order to help you with any nursing interventions. you have given me none to little of that information. telling me that this baby has herpes simplex 2 menigoencephalitis, has been treated with prevacid and acyclovir (these are medical treatments), a low temperature and poor feeding is not enough information to know what is going on here. what does a low temperature and poor feeding mean? assessment information for care planning needs to be specific so that nursing problems (nursing diagnoses) can be determined, goals are customized, interventions can be tailored to what this patient needs and evaluation of the goals can be easily determined.

care planning begins with a good nursing assessment which consists of:
separate out and make a list of the baby's abnormal assessment data. that abnormal data becomes the signs and symptoms with which the nursing problems are identified and named (nursing diagnoses). only then are goals and nursing interventions developed for each of the signs and symptoms. we treat signs and symptoms of nursing problems with nursing interventions just like doctors treat signs and symptoms of medical diseases with medical interventions.
