I am working on a care plan for deficient knowledge regarding STD's in the 15-24 age group. This is part of a community project so there is no actual patient. I did a survey of 20 people in this group. Would the data from that be subjective or objective? For instance, many have had many had sexual partners, some use condoms with other methods, lack of communication with parents, embarrassed to ask questions, etc. What would be observational? Client seems to have low self-esteem? I'm thinking of a senario, why would a teen come to a clinic? Suspecting an STD due to symptoms they have, would that play into this diagnosis of deficient knowledge? I've been working on this for days. Googled, looked at Care Plan books, I am stumped. Any suggestions?
