I was wondering if anyone can recommend a good book that teaches how to write a nursing care plan. My classmates and I are struggling with this since the instructor we had for the class that initially taught this neglected to show us how. I understand that they are not easy. I would just like to find a book or two that walks you through each step clearly so we know what we are looking for and how to phrase each section properly. At this point we are getting basic data sheets and trying to write a care plan without a live patient to assess. I think if we had something that actually explains the steps we would all be better equipped to complete these, I know I would.
p.s. I know each school's plans can be different, I just want to know how to figure out what I need and how to narrow it down. I tend to be too broad as I start overthinking things a LOT.
