My patient was a 93 year old dementia patient. Could someone give me 3 physiological, 1 safety and security, and 1 pyschosocial possible general nursing diagnosis for a dementia patient? Thanks

dementia is a medical diagnosis. nursing diagnoses are a determination of the patient's nursing problems. that is done by first doing an assessment of the patient. assessment consists of:

  • a health history (review of systems)
  • performing a physical exam
  • assessing their adls (at minimum: bathing, dressing, mobility, eating, toileting, and grooming)
  • reviewing the pathophysiology, signs and symptoms and complications of their medical condition - read about dementia here: http://www.merck.com/mmpe/sec16/ch213/ch213c.html
  • reviewing the signs, symptoms and side effects of the medications/treatments that have been ordered they are taking
