This is the first care plan I have really struggled with.
I have a patient who is primarily non responsive. He will occasionally acknowledge you and does groan with movement due to pain.
I think his primary diagnosis should be nutrition related because he has been unable to eat. He is on IV fluid. We have to have 5 interventions for each nursing dx and I'm having trouble finding ones that could fit with him.
I have to have three nursing diagnosis - I'm not sure what the other ones should be?

What is the primary diagnosis for the unresponsiveness/ altered mental status?
Other diagnoses?
Current way he's fed?
Medications?
Mobility?
I'm glad to help, but need a bit more information :)
I PROMISE, there are a LOT of nursing diagnoses that would be good for someone who is unresponsive, has an IV, doesn't communicate, has pain, etc.
