care plan evaluation

i am having trouble with the evaluation of a care plan. i am doind my care plan on relocation stress syndrome. how do i evaluate. some of these interventions: explore what is most important to the person, encourage expression of feelings. i have read my nursing book perry and potter and am even more confused:crying2:. can anyone help me?

part of of your confusion is that you're starting with a diagnosis and then wanting to know what to do about it. that's backward. you develop a diagnosis from what you assessed.

did you have a patient with relocation stress, or did you pick that one out of a list and decide to do it?

if you diagnosed relocation stress syndrome in someone, you'd have found some defining characteristics and related figures.

even if you just picked it randomly... when i look at my nanda for relocation stress syndrome, there's a long list of them. once you have those, you can figure out which of those related factors are something nursing can do something about. does that make sense?
