care map for congestive heart failure pt

i am a rn student and need help with my pt on clincal that is CHF, doing a care map on impaired gas exchange and fluid volume excess, and skin integrity impaired. does anyone have any examples of concept maps that i could veiw. Please email them to me. thanks.
