If you are having health problems, you probably should provide notes from your doctors indicating what specifically is going on and what treatments are needed. Family problems are not as forgivable, depending on what they are. You are responsible for handling personal issues like that on your own time and not letting that affect your work schedule. We all have conflicts in our family from time to time, but are still expected to show up to work, because people are relying on us. I'm not sure how many shifts you are cancelling, but if you're missing a large amount, it is understandable why they won't schedule for anymore. Not knowing the details, your best bet would be to go to the meeting your manager is requesting and explain what is going on. If this has been going on for months, it may be best to look for another job. It is hard to keep someone on schedule when they aren't showing up for their shifts. Remember, someone is likely covering for your missed shifts, and they have their own problems, too. You may have been there for 22 years, but if your work habits have changed significantly, they are not obligated to keep you. Best of luck to you.

Sorry, but if you are cancelling shifts at short notice I would stop giving you shifts. We had a nurse do that to us a few years ago, we just stopped using her.
The meeting with the manager should be to discuss what is happening and give you a chance to put your side over. It can also be used to get you help with your health problems. Take your union with you.
