Can I follow up on a QRR?

i had a patient who had a string of...let's say substandard nurses...who didn't do their jobs, meaning some things were done which should not have been done, and some things were not done which should have been done.

(i can't go into specifics for fear of repercussions.)

anyway, after i ironed out the stuff, i wrote a qrr...and got called in and chewed out by the supervisor for writing the qrr...and the higher-up started ignoring me/being curt with me.

i could not ignore the problem as the patient could have been killed because of incompetence and/or negligence.

the problem is, the supervisor and the higher-up are not happy with me now--and they may have spread the word that i wrote a qrr...meaning i have some co-workers mad at me.

the other thing is, i have a nagging feeling the higher-up might have "buried" my qrr, as 2 of the nurses involved are the higher-up's staff, and are good at kissing bottom. really good, as i finally realized--they supposedly have more than 10 years worth of nursing experience each, but i was shocked at their negligence. (now i understand how they can go home at 7 on the dot.)

my question is, can i follow up on the qrr?

i think the level of error is too much that at least 2 of the 3 should be fired...and at least 1 of them should be reported to the bon.

in retrospect, i think my superiors are acting this way because if the qrr makes its way upwards, it's gonna reflect badly on the floor. but if i inquire about the status of my qrr, it will definitely reach the ears of my superiors.

what's a guy to do?

to put things in perspective, i need my job.

Can you expand upon the meaning of "QRR"? I am guessing from your post that it is some sort of incident report ?

I don't have any advice except to keep careful documentation of all of your actions and those of your superiors in case you need to take this to HR or the DON at some point. Conduct all interactions in writing if possible, or have a witness present.

they know what they are doing. nothing in writing--verbal reprimand.

no witnesses--"can i talk to you for a minute?" in an otherwise empty room.

they know enough not to leave any evidence--which is why i feel the higher up might "lose" the qrr i wrote.

QRR forms are numbered, and they are supposed to be kept in one place and used sequentially. If any are missing they are supposed to be accounted for. You can follow up to see if it was received by whatever department they're supposed to go to, but you won't have access to knowledge of any outcome.

the qrr forms have numbers, but they are distributed to the floors, so the qrrs filed are not essentially sequential. i have also seen nurses discard qrr forms when they make mistakes in filling them up.

the higher up has the authority to override the qrr--or so this higher up said. as long as "it's dealt with."

it's just horrifying to realize that some nurses are very negligent...and get away with it. with help from the people in supervisory positions.

the trick is how to ask if the qrr was received--without alerting my higher ups that i was checking if the qrr was really filed.

My experience has been that the hospital is responsible for being able to show that QRRs are taken seriously, hence the sequential numbers. And all of them have to be accounted for. Sounds like you need to find another employer
