Greetings:
Never have I imagine starting my own business would be so hard. As a nurse I have joined many nursing boards to network as well as non nursing boards. I am one of those persons that believe in not reinventing the wheel. I have went to search engines to look for blank subcontractor forms. Or,,, client service contracts. NOTHING.
Does anyone have any plug and play forms that they have come acrossed during your research?
I have received some forms from Ga State DORS.
But I need.
1. Client Services Contracts
2. CNA/LPN daily visit sheets (flow sheets and/or documentation)
3. Care Plan
4. Assessment Form
5. Medication Profile
6. Sub Conntractor Contract

I too have been looking for client contracts and formats for fee schedule - I;m and indep. Case manager managing W/comp, disabilities...I've had not luck?

I see/hear/feel your pain. Starting up ones own business is a whole new ballgame, isn't?
I get my templates from the professional organization of my specality area. I wish I could help ya'll.

Though not nursing related smartbiz.com might be a good start for the more general forms required. They're free, and there are a lot of really good forms there for the startup and cash strapped entrepreneur. :)

dont walk, RUN, to your nearest SMALL BUSINESS DEVELOPMENT CENTER.
It is part of SBA and usually part of the local university.
They will help with business plans, state taxes, loans, etc, etc and it's all FREE
