Hey, everyone I am at a lost,at this moment in time, You see I was dismissed for Nursing school due to failing to Identify patient during clinical training and failing to accurately take vital signs. However, I did practice several time after the first sims lab and took vital signs correctly. Yet, on the last sims lab my vital signs were incorrect from the instructor. I know how to take vital signs and to ID a patient before giving meds. I do not know why I failed to ask the other student their name and date of birth before giving the medication. I did introduce myself and ask the pretend patient their name on the first look assessment. So I know my patient. Any suggestion will be greatly appreciated, I am so disappointed with myself.

You don't want to hear it and that's fine. You may "know" what you did, but you are not actually acknowledging the issue. Hence why I do assume you had other problems as well.
Sometimes looking in the mirror can be hard. But you need to if you wish to continue on and become a nurse.
I never once insinuated anything about any errors I have or have not made. I simply addressed yours seeing as this thread is about you. Pointing fine gets at me because you are not wanting to admit you are unsafe is not my problem. It is your. But FYI, "friend", you did not in fact mention this happening more than once in your OP. Every time you responded, you added another element to the story.
Just because you had identified the patient earlier in the scenario does not get you off on forgetting a vital step of the rights of medication administration. Yes I have made mistakes. I am a final semester nursing student and have done things wrong throughout my education. But I can honestly say that none of them have been major safety errors, and I haven't repeated the same error multiple times. As you have learned the hard way, major safety errors get you dismissed from programs. Especially when it is not learned from and then repeated 3 times.
How can the instructor trust you to safely pass meds to a stranger in clinical if you can't do so in sim lab. Yes it is a safe environment, but that doesn't mean you get to be lax about it. How many chances do they have to give you to do things the right way. Your education can't progress if you don't learn.
