Assisting patients with change of physician

Hi all! I work as a home dialysis nurse for a company that contracts w many different physicians and practices. Sometimes patients wish to stay w our company/nurses but are unhappy with their physicians and want to change. The various contracted physicians do not play well in the sandbox. I have always been told I'm an advocate for the patient. But, am not allowed to offer even a modicum of assistance w this transition. What is our role as nurses in this situation?

If the patient requests information about changing physicians, I would explain how they can go about initiating that change. You are simply providing information about the process that the patient may act on, should they choose to do so, not assisting with any part of the transition.
