assisting living facility

hello my fellow nurses! let me start of by saying this is my first time using this site and im pretty excited! ok to cut to the chase, i have a resident that has a history of cancer and is currently doing chemo. so nausea is occuring greatly in this person. However, while making a nursing note, i want to chart that he appears very tired and just depress ( im aware this is normal due to going through cancer) and is reporting alot of nausea episodes throughout the shift..lets say more than 10 times. OK so my question is how do i chart his appearance and his emotionally side?

Document what you see: resident with little energy, frowning, furrowed brow...constant nausea

Something like that...but if you chart that, make sure you chart an intervention.
