Applying for licensure and to take the state boards

On my application for licensure, I am being asked if I have any physical or mental impairment that might prevent me from performing my duties as a CNA. I was " diagnosed " with depression as a teenager. Do I need to disclose this? It won't prevent me from performing my duties, as it has been five years since And I have been managing just fine. I'm not on any type of medication, I'm not even in talk therapy although I feel like I should be for other reasons unrelated to my mental status. I'm kind of freaking out right now because that prevented me from joining the military, now all I have is nursing and if that's going to ruin my career I don't know what I'm gonna do.
