Is it a good idea to apply for a job before I graduate? I'm 3 months away and the PERFECT part-time, 20 hours a week job has come up, which I haven't been able to find anywhere! Should I apply now, and call/apply again after graduation? Or is that a silly mistake?

No one can hire you as an RN until you are licensed. I see no point in applying now for a job that will be gone by the time you qualify for it.

How is it possible that some people graduate with jobs (some of them don't even already work for the facility)? Do these places hold the job openings or could it be that the student (s) networked with the right people. I always wonder about this when I hear about it.

I'm a month away from graduation and I've already been offered a job on the PCU I'm doing practicum at. I've also applied to the hospital system itself for all of it's new grad openings (they do a bundle application). They will and do hire you with the contingency of you graduating and passing NCLEX. You must be in your last semester though to qualify for that. They do hold the position for you, keeping in mind, it's a GN position, not your average RN position, so it's not like there's a giant hole on the unit for a month or so while you wait to test.
That's how it is in my area at least.
ETA: I wouldn't apply for that position if it's not a new grad position. Like the pp stated, a normal position would most likely not be held for you for three months or longer with you having to wait to test.

I 100% would apply. If you are selected to interview it will be easy to explain that you will be graduating soon. If you are the right candidate for them they will wait. I graduated with a job offer back when I was a new grad, and I started applying a few months before I graduated.
