anyone have a good report sheet specifically designed for neuro?? i have made one up as the one supplied by my facility was designed by someone who writes in a 4 point font; i need more space. even if yours is small, i would love to see it to get a better idea of what i need. thanks!

Could you upload the brain-sheet you have? I'm interested in what it would look like for a neuro floor . . . I tried to find some online, but only found the generic med/surg report sheets. However, I found a great powerpoint that describes a detailed neuro assessment at:

what i have is very specific to the rehab hospital that i am working at and is still a work in progress. i will work on it over the next few days and post it here. as i am new and will be going into a new tbi rehab unit, i have little idea of what aditional stuff should be on the report sheet; so what i have may be entirely off the mark.

i am a new nurse also, so i am looking for a good one too! the one i received from my preceptor is good, but i want one on the computer that i can toy with. i'd show you hers, but it is just a copy of a copy and wouldn't scan! if anyone finds one, i need it too!
